Experience & Expertise
Sovereign Nurses have a minimum 5 years experience in In-Home Care.
Sovereign Nurses are experienced in specialised treatments such as catheter management, dementia related behavioural support, complex wound management and palliation.
Different needs may require referrals from different agencies.
Nurses may not have significant experience in the home environment or the ability to respond to complex issues. Multipe referrals or agencies result in wasted time and funds.
Reporting
Our standard practice is to deliver reports within 24 hours of the visit.
Our reports are designed to give non-health trained coordinators and caregivers clear recommendations to support the individual outcomes of the client.
Reports are often under 25 words or not delivered.
Record Keeping
Our Registered Nurses provide a full electronic health record for each client.
This enables our nurses to track health trends and provide timely support.
Minimal record keeping or client information is provided.
Information provided may be as minimal as the client’s name and address.
Consistency & Continuity of Service
Our nurses are organised into districts and matched by skill set for each client.
This enables nurses to build a relationship with their client and to deliver better outcomes.
There is no consistency of the individual nurse or their skill set.
Clinical Advice
Sovereign provides in-depth, personalised clinical advice.
Sovereign Nurses provide advice that takes into consideration the client’s health, living and funding situation. This is made possible by ongoing exposure and relationship with the client, collaboration with coordinators and detailed record keeping.
Limited advice can be given by a nurse.
Limited advice can be given due to limited client information or exposure.
Support for Provider’s Clinical Governance processes
Our nurses hold regular meetings with providers.
This enables nurses to support and maintain the individual needs of their clients.
Meetings can often not be supported.
This may be due to a lack of capacity, consistency of staff or available information.
